Most frequent questions and answers about the North Halton OHT

An Ontario Health Team is an initiative by the Ministry of Health that was designed to remove silos and re-design care around the patients and their care givers.

The North Halton OHT is led by primary care physicians working here in our community.

Currently, our partners are the Mississauga Halton LHIN, Halton Healthcare, multiple community organizations and private health care delivery companies.

We are currently designated as “in-development” stage by the Ministry of Health.  We are awaiting the Ministry’s response in March 2020. In-development means that we have demonstrated “a strong degree of readiness to implement and represent a continuum of care”. Fraser, M. (2019, July 18).
